What is 1-2-7?
1-2-7 provides a realistic, time limited approach to safety planning and focuses on fostering empathic conversation which engages the person and when appropriate their family in problem solving and thinking around scenarios which may occur within the first week after a mental health crisis or discharge from inpatient stay. It is a semi-structured conversation guide. The prompts are framed around daily events that matter to people. 1-2-7 begins with a focus on the first hour post discharge and the pragmatics of what may happen when a person gets to where they stay. It then establishes how the person will occupy themselves over the following two days and if they have the necessary supports. The conversation then turns to how the risk may fluctuate adversely over the coming week and how some of these risks can be mitigated. The goal is to identify triggers, early warning signs, internal coping and socialisation strategies, reducing access to means and using family / professionals to navigate crisis.

- SAFE - SOLVE: OverviewThe questions in 1-2-7 start up the curious compassionate conversation, remember that you need to listen out for a whole lot more than the literal answer.6
- One Hour: Pragmatics in the Here and NowThe three questions in this section all relate to the here and now. They are designed to exhibit genuine curiosity and build on the rapport that should have been established during the history taking process. In some cases the decision to send someone home may leave them feeling unheard and invalidated. An authentic exchange can then help reveal and / or heal any fractures in rapport that might have developed. Enquiring about simple logistics may not seem much but they are far reaching in conveying that the professional does care.12

- Two Days: Amenities, Time, CompanyThe set of three questions for the 2 day section of 1-2-7 are at the heart of the shift from what’s the matter with you to what matters to you. Irrespective of whether a person has mental health challenges or not, there are commonalities in what matters to people. They are captured in Maslow’s hierarchy of needs and the questions show curiosity about the basic amenities of life, things people want to do and who they want to do it with.9

- The first weekMost developed countries have now established a system of 7 day follow up to account for this period of elevated risk. However, it does not address the intervening time window. The purpose of these three questions is to mindfully establish exactly what might tip someone again into suicidal distress, reiterate easy solutions and how to access help.5
